FOR PAST PATIENTS
CONSULTATIONS
I am no longer taking consultations, and I am not issuing new prescriptions or repeat prescriptions. After many years of seeing patients, I have stepped back from clinical practice in India in order to teach and to write. Menopause care in India needs more clinicians who understand it, and that is where my work is now.
IF YOU ARE ALREADY MY PATIENT
I have written to each of my patients on treatment individually about who to see next. If you are my patient and you have not heard from me, please write to me at drsukhpreet@menopausewize.com.
If I have prescribed for you, that prescription is valid for the period written on it and no longer. Please arrange to be seen in
person by a registered medical practitioner, ideally a gynaecologist or a doctor who practises in menopause care, before
it runs out. Take your prescription and your most recent test results with you.
Please do not continue beyond the period written on your prescription without being seen.
IF YOU FEEL UNWELL
Please do not wait to hear from me. See your nearest doctor, or go to your nearest hospital. That is true of any symptom that worries you, and particularly of unusual bleeding, chest pain, breast pain or changes, breathlessness, severe headache, or swelling or pain in a leg.
IF YOU ARE NOT MY PATIENT AND ARE LOOKING FOR A DOCTOR
I am not able to recommend a doctor to somebody I have not treated. Your own gynaecologist or general practitioner is the right place to start. The Indian Menopause Society publishes a directory of its members, which is maintained independently of me.
YOUR MEDICAL RECORDS
Your records are held securely and are not affected by this change. If you or a doctor you are seeing would like a copy, write to me at drsukhpreet@menopausewize.com and I will send it. How your records are held and for how long is explained below .
IF YOU WOULD LIKE TO KEEP HEARING FROM ME
I write about menopause at The MWize Life. That is education and general information. It is not medical advice about you and it
is not a consultation.
YOUR MEDICAL RECORDS: HOW THEY ARE HELD AND FOR HOW LONG
This notice is for patients I have treated. It explains what I hold about you, why I still hold it now that I am no longer in clinical practice in India, how long I will keep it, and what you can ask me to do.
1. WHO HOLDS YOUR RECORDS
I do, personally. You can reach me at drsukhpreet@menopausewize.com
2. WHAT I HOLD
Your consultation notes, the summaries I sent you after each appointment, prescriptions I wrote, any test results you sent me,
and our correspondence about your care. I also hold your name and contact details and a record of your appointments and payments.
3. WHY I STILL HOLD IT
For three reasons, and the third is the one that decides how long I keep it.
3.1 So that you, or a doctor you go to next, can be given a copy of your records when you ask for them.
3.2 So that I can answer any question you have about the care I gave you.
3.3 So that I can account for that care if I am ever asked to, whether by you, by a consumer forum, by a court, or by a Medical
Council. A complaint about a doctor's care may be made long after the treatment, and in the case of a Medical Council there is no
time limit at all. Keeping your records is the only way I could answer such a question fairly, to you as much as to me.
4. HOW LONG I KEEP IT
Ten years from our last consultation. Where I wrote you a prescription, ten years from the last date on that prescription, if
that is later. I keep some records for longer, without a fixed end date, where there was an unexpected outcome, where a concern of any kind was raised, where a treatment decision would need explaining, or where we lost touch while you were under treatment. In each of those situations the record is the only account of what happened and I do not think it right to destroy it.
I keep your appointment and payment records for as long as tax and accounting rules require.
When a record is due for destruction I destroy it securely and note what was destroyed and when.
5. WHERE IT IS HELD
Securely, on services that may be located outside India, including in Canada. Wherever it is held, it is protected by the same
measures and is not available to anyone else.
6. WHO ELSE SEES IT
Nobody, unless you ask me to send it to a doctor or to someone else you name, or unless I am required by law to produce it.
I do not use your information for marketing, I do not sell it, and I do not give it to anyone for their own purposes.
7. WHAT YOU CAN ASK ME TO DO
7.1 ASK FOR A COPY. Write to me and I will acknowledge your request and send your records within 72 hours. There is no charge and you do not have to give a reason.
7.2 ASK ME TO CORRECT SOMETHING. If a factual detail is wrong, write to me and I will correct it. Where the entry is a clinical
note made at the time, I will not delete or rewrite it, because a note made at the time is evidence of what was recorded on the day. Instead I will add your correction to the file alongside it, with the date, so that anyone reading the record sees both.
7.3 ASK ME TO DELETE. You may ask, and I will do it unless keeping the record is necessary for the reasons at paragraph 3.3, in which case I will tell you so and tell you why. I will explain rather than simply refuse.
8. IF YOU ARE NOT HAPPY
Write to me first, at the address above. I will acknowledge within three working days and answer within fourteen. If you are still not
satisfied, you keep every right you have under the law that applies to you, and nothing in this notice takes any of them away.
This notice applies from September 30, 2026.
